Can anyone please tell me what community-based geriatric care is?

I would tend to disagree with that. WHile some community-based nursing care happens in the home, alot of it is based outside of the home. THere are numerous community health centers. They often diabetic/nutrition counseling, BP clinics, blood glucose monitoring, flu shots clinics, medication help, etc. The local office of aging helps to get meals on wheels, legal advice as well as homemakers(shopping,cleaning) and CNAs in the home. Community-based means found in the community. While the source of funding for community-based care may be in part Medicare, it is often through local/state agencies
Home healthcare does service seniors but it is not a senior-based nursing service. My caseload at present has a kid, a few teens, quite a few patients in 20s-50s as well as handful of seniors - all receiving very different things (IVs, wounds, trachs, catheters, bloodwork,etc). We often refer seniors to agencies for community-based care because Medicare is very strict on what it covers - skilled medical needs only. Anyway, just my .
It undoubtedly depends on where you live as to what different people call it.

As the population (read community) ages- I guess we'll see more of it- whatever it is!!!
