UTI Nursing Diagnosis help!

My patient came in because of altered mental status and left lower extremity pitting edema. while in the hospital they discovered he also has a UTI. We also found out that he was hoarding his meds in his mouth so then they nurse would crush them or open then pills and put them with applesauce only do discover he would not swallow that either. He was there for 3 days and already d/c. I'm needing three nursing dx's for a concept map and have only come up with 2: Risk for impaired skin integrity and Communication, verbal, impaired. I'm not sure what to put as my third dx...something involving the UTI or his refusal to take meds? I'm new to the whole nursing dx thing and any help would be appreciated!

any diagnosis really has to be based on any assessment data you found and you really haven't provided much, if any, assessment information. what were the symptoms of the uti? what medications were being given? was the patient incontinent? when patients are not cooperative with treatment it is generally because they are confused or noncompliant. there would be symptoms of this. was this patient confused (chronic confusion)? patients who purposely do not do what the doctors want have the problem of ineffective health maintenance if they are not confused and have other reasons for not following the medical plan of care.

nursing assessment consists of:

  • doing a health history (review of systems)
  • performing a physical exam
  • assessing their adls (at minimum: bathing, dressing, mobility, eating, toileting, and grooming)
  • reviewing the pathophysiology, signs and symptoms and complications of their medical condition
  • reviewing the signs, symptoms and side effects of the medications/treatments that have been ordered and that the patient is taking

diagnosis is then based upon the abnormal data that is collected during the assessment. abnormal data is matched with the signs and symptoms of nursing diagnoses that fit with what is going on with the patient. a nursing diagnosis is merely a label (name) for a problem going on. you need to read the definitions of the various nursing diagnoses to better understand what each of them mean. you can find them in a nursing diagnosis reference manual. the appendix of taber's cyclopedic medical dictionary contains this information.

Hi, I'm new to all nurses so i'm not exactly sure how to start a new thread to ask my question..

I have an 80 yo female patient. She was admitted for hypoglycemia, they also diagnosed her with a UTI. Her blood glucose on arrival was 20. She was disoriented x3. had visual hallucinations, she has COPD--during morning meds she look at the inhaler like she had never seen it before in her life. unable to remember how many children she had, waiting for her son to come (he had already been to visit earlier in the afternoon) her urine bacteria level was >100,000 ml/col

would this work as the nursing diagnosis? i've never done secondary to in my diagnosis before so i'm not quite sure how they work

Acute confusion r/t delirium secondary to UTI infection and hypoglycemia

hey daisy, am also new, but i figured out that by goin to a specified subcription on top will give u an option for new thread.

as for your question am just first semester nursing student so please dont take my word for it and consult someone who is an expert in this. from what it looks like i think the major concern will be to work on COPD so probably ineffective breathing pattern?

another one could be impaired memory.

I have ineffective breathing pattern and acute confusion (great minds think alike). For our concept map we need several diagnoses so I am just trying to come up with some other diagnoses.
