Anyone applied for their FNP program for Spring 2021 start? 

Hello all!

Did anyone else have difficulty locating the COVID-19 Attestation form?  I have been unsuccessful in trying to find it.  Can anyone kindly point me in the right direction?  Thanks so much! ?
