Triage Form

Hey everyone, I am a fellow CDN nurse, new to this discussion board but not new to nursing. I am working on a new flow sheet for our Level 1 Trauma Pediatric ED. Wondering if anyone who may work in the ED would have time to scan or copy the triage component of their ED flow sheet and fire it off to me via email/fax etc ? We are trying to improve our current form and have recently adopted a lot of the fairly recent CTAS standards for triage. Thanks for your help.
