Trauma Cart?

Yesterday, our rural ED had a bad MVC in which a pt. ended up getting RSI. The Pt. lost pressure and a pulse in which a thoracotomy was performed. We have to get meds from a pyxis type machine and pull each med ind.= time. Our thoracotomy tray is seperated from our spreader = time.

Although outcome would have not changed, do most ED/trauma 1 have RSI meds at hand in a bag with a Cart?

Do you have a cart that everything in it? Airway, Cric., spreader, gowns? and if so what exactly is in there?

We have a trauma cart in our trauma room. Everything possible on it. We are in the process of updating our room so we just finished visiting some area hospitals to check out their trauma rooms. Most have some verson of a trauma cart.

We have drugs on our crash carts and in a pyxis. Our crash cart has emergency meds...not just ACLS meds. Also, we have vecc and succs on our crash carts in the ED. For codes on the floor we have a packaged RSI kit in the pyxis frig.

All of our needed supplies are there, we also have an intubation cart in the biggest trauma bay with everything needed for RSI on it (except meds) Our crash cart is equipped with ACLS drugs but not succ or vec or versed. we just "John Doe" the pyxis and remove the drugs when needed. Takes less than 30 seconds.

we are a small 36,000 per year ED and we have 2 trauma rooms. these rooms are supposedly stocked daily but we also use those rooms for simple cases. i've noticed we do typically need 'something' when we have a trauma that isn't in the room.

our acls drugs are in a locked cabinet in each room. we have locked MI boxes in the rooms. our RSI kits are in the pyxis and we either pull it out under john doe or EDstock. takes 30 sec. can be done as squad is rolling in the door. the kits are then sent to pharmacy to be refilled and charged out to the patient's account.

Our crash carts have intubation boxes on them. They are tackle boxes with the laryngoscope handle and blades of every size and type...ET tubes, stylet, syringe and ETT holders. One crash cart is kept in our trauma room, one on the unit, one in express care and a peds crash cart in our peds trauma room.

The only drug I usually have to run for is the versed...and we are working on changing that. The crash cart drawer that has our emergency drugs on it are in a "sleeve" that pharmacy has pre-packaged and they replace the sleeve at every code. It works pretty well.

We want distro to do the same with supplies...

I am finishing placement in the ER in our regional health sciences centre.

We see approx 240 patients a day. We have 4 Trauma rooms. 2 are designated for resusc and 2 for traumas.

all 4 rooms have their own articulating arms that have our ambu bag 4 suction sources and 4 O2 sources on the arm. A vitals monitor, then it has various airways and instruments set up for intubation and opening airways. then our back tray carries various stuff, such as N95 masks, sterile gloves, gowns, closed chest drainage trays, central line insertion kits, suture kits, paracentesis trays, catheterization kits, along with numerous other kits and instruments.

Each room has a crash cart that carries a large amount of drugs for resusc, ACLS and Trauma, as well as preloaded syringes, so we do not have to run for any drugs, and a biphasic defibrillator.

Each room has a dual bag pressure infuser

The 2 trauma rooms have an inhouse X-Ray machine and portable ultrasound as well.

We also stock all the RT equipment the RT's would need, and Iv equipment in room.

WHen a trauma comes in, we do not have to leave the room for anything, until either the patient hsa to go for a ct scan or is going to ICU or OR.

Our pediatric resusc rooms are set up in the same manner but everything in the peds rooms works on the Braslow system.

We keep both an adult and ped crash cart in our trauma bay, we also have a small locked refrigerator w/ 3 "bags" of RSI kits...In our main department we have 3 pyxis machines and each has at least 2 RSI kits in them. We have 4 crash carts and another peds crash cart in our main dept...
