Why are Minimally invasive Aortic valve replacement patients at higher risk for asystole postoperatively? They have their pacing wires intact up to 72 hours post op which is longer than other cardiac surgical procedures.

The cases I've seen, they don't remove the old value. It's not difficult to compress the conduction system or have swelling. My understanding is that heart block is the most common conduction issue, thus pts are fitted with pacemakers or wires even before the actual TAVR.

I completely agree with you. They are trialing a new core-valve called the Evolut that has been re-sized and gives you the option to re-position the valve giving a better anchored during deployment of the valve. This decreases the chances of depressing the purkinje system. The patient's with these valves come out without pacing wires and do well.
