Our facility has had a few med errors with removing transdermal patches or should I say "not removing".

How do other facilities document patch removal??

What info do they actually document on MAR?

Its a problem too in my facility (LTC). Our order reads Nitro patch on in am and off at hs and the times are marked for 9a on, 9pm off. nurse is to inital them when place on a place is marked for them to note the site.

Cant tell you how many times I go to remove them at night and find a few on them. What kills me is that some are dated, some are not and they even put them on next to an old one. :angryfire
