As announced, I took my NCLEX today; for the third time. I walked out of there feeling pretty good. But then reality set in: remember the questions I debated about.
The computer shut off at 116. Way different than my previous two experiences, where I had 265 each time! I feel very confident that I got my last question correct. I also noticed that throughout the last half of my test I got many questions like: "Who would you see first, Who would you discharge first..." I was told that was a good sign. I never have gotten as many of those in my last two sittings as I did today.
I leave you all with some questions:
A patient received a Bone Marrow Transplant 2 days ago. What would you want to report immediately?
A. BP of 98/60
B. Temp of 37.2 (97.2)
C. Nausea and Vomiting
D. Cough with Yellow Sputum
Which would you want to carry out for a comatose client who is to recieve an intermittent NG feeding?
A. Lay then on their LEFT side
B. Hang the bottle higher than height of the stomach
What would be an act of negligence?
A. Filling out an incident report about a client's fall and leaving it on the outside of the chart
B. Telling a client with a head injury that their Coma Scale Score went from 12 -> 14
Who would you see first?
A. Someone with a casted Right extremity whom has right edema and decreased circulation
B. A client with a Coma Scale score of 7 and is restless
Who would you discharge first in a disaster?
A. Bowel Resection 48 hours ago, whom is NPO
B. Infant who had a Pylorotomy 2 hours ago who vomited their first feed and retained the second
A postpartum client of 2 days has saturated several pads in 2-3 hours and has not responded to modalities to firm her Uterus; the doctor tells you that this is ok and expected. What is essentail to do next?
A. Tell the charge RN
B. Document the conversation with the MD in the chart
*These were questions I wrestled over and cannot seem to find the answer to once I arrived home.*
