TN Visa "Health Screen"?

I am Canadian, my wife and live and work in Texas. I recently switched from L1 to H1, at that time my wife who was switched H4 had to quit her nursing job.

My wife is a Canadian citizen, we hired a lawyer to prepare a TN package for my wife.

My wife went to Canada an upon reentry was not accepted for a TN visa and not allowed back on her H4.

She was told that her letter of employment must be addressed to "US Department of Homeland Security" (it was addressed to her) and that she needed a "Health Screen"

The hospital where she worked is sending us another job offer addressed to DHS but we do not know what they mean by "Health Screen". Our lawyer thinks they mean Visa screen. My wife presented her Texas Nursing license.

We have have been told she needs either a CGFNS certificate or NCLEX-RN exam but I am confused.

Do we need to get evidence she passed her NCLEX-RN exam or do we need this CGFNS certificate or is it a Visa screen certificate?

Visa screen certificate is probably what they are after as that states pass in either NCLEX or CGFNS exam
