TIME Sensitive Care plan experience Needed...Tanning bed Nursing Diagnosis

Hi! So I'm in womens and childrens and i have to make a care plan for a clinical rotation at a family doctors office.....A 22 yr old came in with yellow discoloration in both eyes on the sclera towards her tear ducts---we think its pinguecala... she stated she tans a lot and doesn't use eye protection and knows its bad for her. She also lost 10 lbs since her last visit and i know her--small town--- and she is definitely way skinner then she has been but she fluctuates... so i have to make 2 nursing diagnosis from that. Im thinking risk for impaired skin integrity r/t repeated exposure to UV radiation and ineffective health maintenance but the nanda nursing interventions don't line up very well with her issues, what do you guys think? This is due in a couple of hours...so quick responses would be appreciated! I was literally brining patients in and out of the rooms all day taking BP, wt, HR and then on to the next pt so i don't have much to use for subjective/objective data

Could you do knowledge deficit? It sounds like she doesn't realize she could permanently damage her eyes by not wearing protective eyewear. That's just one idea.
