time out boards

Wondering if any one uses a "time out" grease board in the or.

If so....where did you purchase the board?

We don't have grease boards, but we do have posters printed by the hospital posted that tell us what must be included.

We have dry erase marker boards, that are mandatory that we write all of our time out info...no idea where they were purchased.

We have dry-erase boards hanging in each OR that we must fill out prior to the patient coming in and then read from during the time out. They are just standard dry-erase boards that you can find at any office supply store and there are labels (patient, procedure, site, mark, allergies, etc) that were made and glued to each board. I know we had them "made," I'm not sure by whom.

We use a regular dry erase board and made labels to place on them. We fill it out with all the time out information before bringing a pt. into the OR. We also have bright orange towels on the mayo stands that say TIME OUT. They are removed once the time out is done.
