I need to take chemistry as a pre-req to SUNY downstate. I am not good in math and am terrified of chemistry. There is no specification in the "type" of chemistry needed. Which kind of chemistry course would you say is by far the easiest kind of chemistry? Intro, Basic, General or are those all the same things?

Those are all the same. Don't worry-I was afraid of Chem as well. I got an A and I am now a Chem lover!!! The math/algebra is very, very, very basic. It will also help ease you into some of the nursing calculations and gets you familiar w/ units.

Be very careful. Some of my fellow students thought that also and did the "intro" only to find that they needed to do another course afterwards. If it is a pre-requisite I guarantee they have a set idea about what kind of class you need to do.
I don't know your school. Are you doing a BSN? My college required a 4 credit general chemistry course for the BSN. If you are doing an ADN make sure you are strategic. Perhaps you will want to do a BSN later? You get my drift. Why not make an appointment with the your college's nursing advisor? They are really best placed to advise you.
As for the fear of the chemistry and math? I feel ya! Get some practice with math from math.com (also something called purplemath.com if you need in depth explanations). Start now and do 15 minutes a day or something like that. Work your way up to and through scientific notation, ratios/proportions & percentages, units & conversions, simple statistics.
Also, I find it best to study my least favorite subject FIRST. That way I am more alert and receptive to the new info. Then I go on to the stuff I really like. This changed me from a barely C student in math and chemistry into an A student in them (I swear).
By the way, when I finish my last chemistry class I practically had a party!

I'll second what DolceVita said. Check with an adviser at SUNY Downstate, they'll likely have a list of course numbers at various schools in the area of approved courses.
As far as freaking out about chem and math....take a deep breath and take it day by day. Use the resources offered at school (math tutors, chem tutors, etc). You can do this! Chemistry can take some work, but it's very do-able. I started out taking a chem class so I could take the upper level electives I thought I wanted.....a year later, I was a chem minor! :-) Really, it can be surprising.

I went to a SUNY upstate school and took CHEM 151 (chemistry for the health sciences). That was ten years ago, so I don't know if they have that class anymore, but it was a great class and not that difficult. The professor made it very interesting and I passed with an A...after almost failing chem in high school several years prior.
