Telephone orders

Can nurse Jones RN who has legally changed her name as well as her name on her Florida RN license decide to now electronically sign her name on telephone orders using her maiden name of nurse Smith RN? Isn't this practicing nursing without a license as nurse Smith RN doesn't legally exist? This has to be illegal right?
