I know it's a minor thing, but I feel like there is never any consistency in the order of how people are introduced in the OR during timeout. Does anyone have a standard order that they follow or do we all just make it up as we go along?

There is not any order in introductions where I work. The surgeon (Resident, Fellow or Attending ) usually leads the time out so they might introduce themselves first, then it typically goes around the room . Circulator, Scrub nurse, anesthesia resident & attending , neuromonitoring tech, visiting med students. If I am circulating I will also ask those who don't introduce themselves, or who are overlooked, to introduce themselves to the team . So, it's typically a random order, but everyone introduces themselves ultimately.
