Teaching teacher about emergencies

I thought there was a thread about guidelines for teachers about what constitutes an emergency call to the nurse. I can't seem to find that. 

What guidelines do you use? I remember someone using "red white and blue" to summarize heavy bleeding, not breathing well, and... white? I can't find that either. (I think it was an article in a journal)
