Hi I am having difficulty with my Care Plans and teaching care plans, Instructor keeps saying is incomplete would anybody give me some feedback here they are. Thanks for the feedback

3/8/13 Outcome met at 8:30 patient was able to talk on the phone with a member of their church about his fears and following that conversation he seemed to be more relaxed and smiling when family members came back to the room". Will continue to monitor
