Syncope/ intractable nausea/ rule out CVA and arrhythmias

I need to come up with key assessments and priority problems for a patient that presented with snycope and nausea. In her chart, that was what was under her reason for hospital stay. I also found a note from by the MD. that said rule out CVA and arrhythmias. I am stuck. My patient has no complaints now and I don't know where to start. What should my current primary diagnosis be?

She came in with syncope and nausea. That is why the MD wanted to rule out CVA and arrhythmias. Syncope and nausea are symptoms of these two things. I would put syncope and nausea as her admitting diagnoses. That is what the coders will do when they bill her insurance (or Medicare).
