Stupid question...

Have you ever been sure about a topic and then someone asks a question that totally makes you start questioning your certainty on said topic? That's the kind of morning I'm having. So here's my stupid question that I need some reassurance on...

For your returning students, do you require brand new medication authorization forms at the beginning of each school year?

I mean, clearly new ones are required for any changes in their medications or emergency plans, but I had a parent ask if they would have to fill new forms out if nothing is going to change for the next school year.

I knew I was right! I had a parent look at me like I had 3 heads when I told her that we would need new forms at the beginning of next year. Ugh. I hate when people make me question myself. Especially when in reference to my job. Thanks Old Dude!

Each school year stands alone, so yes - new forms every single year!!

A lot of kids have their annual PEs during the summer- changes can happen. We also need new paperwork at the beginning of the new school year.

New forms each year here as well.

It is important for liability. If you require a new form then you are requiring the most up to date information. Parents are supposed to keep us updated to changes throughout the year, but that seldom happens. So it's a way to get current information, if they don't keep us updated and something goes awry, then we can't be held responsible for not having the info.

ETA- It's the same principle with any doctor requiring a physical exam each year before renewing prescriptions

New forms, yep. Especially for prescription medication - a doctor's order is only good for one school year.

Same- new forms every year.

Our district also requires new forms at the beginning of each school year. With that being said, if the student is attending summer school, current orders are good through the summer.

And don't feel bad, I just had a parent give me a hard time about it, even though she has gotten the order renewed yearly for the past 4 years, As required. All of a sudden it's a problem. :sarcastic:
