I am doing a careplan on a patient with abdominal surgery for a small bowel obstruction with entero - entero anestemosis and lysis of adhesions. He also had an umbilical hernia. The problem is he has no medical issues or history except for one previous abdominal surgery for a GSW, collapsed lung, fractured ribs, and spleen removal. Neutrophils and Monocytes where high, and lymphocytes low. He was NPO, which I am assuming is the reason his urine was brown, and on bed rest. But other than that everything else was normal. Can anyone help me figure out what kind of NANDA's I can use for my careplan? I am really stuck on how to start it off.

How about Acute Pain r/t surgical procedure..., Risk for Infection r/t invasive surgical procedure...., Constipation r/t decreased activity, anesthesia, narcotics...

Altered nutrition.... because of the NPO.
I would also do a bit more to address the amber urine...
