SOC

Published

i am hoping someone can shed some light on this.

i know we did this in the past just don't know if it is still done

here's the scene;:::

busy day,,,,no extra help,,,,new patient needs to be admitted,,,RN goes out today 11/4 and gets the consent signed, does a routine nursing visit,,,11/5 another RN goes out and does the actual admission paperwork, etc,,,cert period is 11/4 - 1/2..M090 is 11/5......

just wondering if any of you do this when that schedule is tight.

thanks in advance.

Yes. Just be sure that the nurse out doing the initial visit is providing skilled care or you would not be in compliance. But, yes, the initial visit establishes SOC (11/4) and M090 is 11/5 if that is when the OASIS assessment is completed. And the only time we do it that way is if a hospice nurse doing call during the week has to go make a initial visit for TPN or IV, etc..then a home health nurse does the OASIS the next day.

thanks! i thought it was possible, but i can't find it anywhere in the regs, do you have any idea where i might find it in writting,,,seems no one believes it unless it is in writting.

thanks in advance.

I have a book of all the CMS OASIS Q&As to reference. But I was able to find the link online on CMS site(hope it works). It is category 2 - Comprehensive assessments and is question A20. Hope it is what you need

thanks that did work but i am actually looking for is something that says it is ok to send RN out tonight to get consent signed and perform the skill and tomorrow send another RN out to do the actual soc,,,the soc date for cert period would be that night visit and M090 would be that day visit...does that make sense to you. we use to do it all the time at one agency i worked at when we were short handed one day or night and it was never an issue and that agency was very much above board....now all of a sudden the agency i work for says oh yes you can do that but it has to be the same nurse for both those visits i say it doesn't but unless i can find it somewhere .....well you know how that story goes.

thanks again.

I'll keep looking in the regs for you. But the problem with your agency's train of thought is that they are looking at the initial visit and the OASIS SOC assessment as the same thing. They are NOT the same. And I am not sure that it specifically states in black and white "you can send out 2 different clinicians" but when it does not say that you cannot, then it is permissible. But a RN or PT doing a initial visit and providing a skilled service does establish SOC, whether or not OASIS is done or even started. But, I understand what you are saying. I think that the link or looking at COPs will say the same sort of thing often enough you can prove your point.

In the link I posted earlier - look at question A12. The last sentence basically says that a different clinician from the one who provided intial visit and established SOC is able to perform OASIS within the 5 day period
