Generally a person has more say if they do extended care cases. If you do intermittent visits, well, somebody has to do the visits and if you aren't willing to go to certain neighborhoods or travel all over the place, then they won't have much use for you. On the other hand, if you do extended care, you can refuse a case that is in an area you don't care to go to or too far away or not the hours or days you want to work. You can do this to a point. If you are too picky they will give the available work to someone else. Shift work is typicaly 8 hours, although shifts can vary from 4 to even 16 hours, depending upon the approved daily hours and available nurses. You can even work out unique schedules with the client and the agency will usually approve it. Sometimes nurses will work a day shift but leave at two instead of three to be home for kiddies or because they need travel time to get to a PM shift job. Or a day shift can start at 5 am instead of 6, 7, or 8. It depends. That is why agencies like to tell you that home health is flexible. If you do shift work, you will have sufficient time to complete your nursing note in the home. If you do intermittent visits, you may or may not, find yourself doing paperwork at home. That is a benefit of doing shift work instead of intermittent visits.
