I'm looking for other school nurses from Illinois. What do you think of the new physical form. I received a copy via email this week.

Contains great info, but will it fly? Would like to hear from others.

I think it covers a lot more. I'm going to use it as a front and back instead of two pages. I don't see as we have a choice but to use it since it is state mandated.

You are right - we don't have a choice. My concern is regarding physicians or nurse practioners who only complete the bare minimum. I can't imagine everyone taking the time to complete it.

Another concern is the portion required of parents. Some of the parents I deal with would be unable to complete the history portions.

I suppose what we get is what we get. It will definitely be interesting.

The parents never completed the last part of the physical form either. I always had trouble with that.

I'm not a school RN, just the mom of a child about to enter Kindergarten. I had an appointment for her 5 year-old/school physical last month, and happened to be at the school the day before, so I stopped in the office to ask for a copy of the physical form. The secretary mentioned that it was a good thing I had asked, since they had just received the new form from the State of IL that week, and were told that they were not to accept ANY physicals done on the old form.

I can't help but wonder what will happen with the children who had their physicals done early. The staff in the peds office asked to copy my form, since they had not received any information form the State about the new form yet. Do you suppose the school districts will just accept the old form for physicals already done, or will the peds offices have to re-do all the old forms they've already filled out? What a pain?

It is my understanding that we can accept physicals on the old form until January 2004. We destroyed all old forms and have been using only new. There aren't that many "old" forms out there anymore so it shouldn't be that much of a problem.

All of our physicians are stilling using the old forms. We send them copies of the new forms but they continue to use the old ones. We just accept them and work on change. Some of these physicians are still using the form that came before the new form. It's like pulling teeth to get cooperation from some of them.
