For the degrees, you get points for one degree, either a bachelor's or an associate's. Of course, I would recommend putting the bachelor's so you get the 5 full pts, and the degree does not have to be related to health science; it can be any degree, such as a degree in art.

I only hold two associates degrees: one in math & science and the other in health science. I am putting health science because it's shorter to spell out LOL I'm paranoid about writing anything the wrong way and getting disqualified. It doesn't have to be the degree that pertains to this field as long as the degree is completely and posted on your transcript, that's all that really matters.

I would go with whatever is on the report since that's on paper, and that's what they are going to need documentation of. If you don't have any papers that say the 22nd, then I wouldn't do that; I would go with the 23rd since you have documentation for that.
