San Jacinto College ADN 2014

I'm in the process of getting my application ready at San Jacinto College (Houston, Tx) I have most of the things ready i just need to take my Hesi, which will be this friday! I only had 3 1/2 wks to study but I mainly focused on A&P and Bio the rest should be easy for me, but i'm still a little scared. However, i have some questions that i'm still a little uncertain about 1. For the immunization i know they want copies but they said no immunization cards? so what exactly do they want? For those who are already in or submitted the application what copies did you turn in? When i went to my Dr. he just made a copy of my Immunization card. 2.Do you have to take a physical or is that till you're in the program already?

I went ahead and took one just in case but my Dr. did not give me any copies of it so just want to make sure if we do or don't need a physical so i can go ahead and go and ask him.

I am currently in the LVN program for San Jac and they use a program called certified background that cost like $100, but on that site you upload all the info like immunizations and cpr, etc. So I don't think it matters if its original or copy. I took my physical a week after Orientation because they gave me a sheet for the doctor to fill out.
