I've been given the assignment of creating new medication charts for our combined ICU/CCU/PICU ward. I could do with some good tips, who has come across a really good @ safe system for medication. How do you solve problems like; Medication for long time stay patients - do you transcripe once chart is full? Verbal orders? Dilution for paeds doses? How do you order the medication from pharmacy? Illegible doc's handwriting? How to clearly show the times medication is due? How to prevent errors in way of giving (oral/iv/im etc)? Looking forwards to your tips. Thanks, Nurina

The best system seems to be computerized charting and BCMA (bar code medication administration. As you are working on your project, you might want to push for a possibe conversion to this system as it greatly reduces the confusion and amt of errors (transcribing, illegible handwriting, etc). Cost is of course, the greatest factor, and research into this would greatly benefit your proposal.
