Hello all,
I am a little confused about how to formulate a statement under the level 2 assessment for the roy careplan, the reason being, isn't it kind of the same as a nursing diagnosis?
Ex.
Level 1
Client has a weight bearing as tolerated on her right side.
Stimuli
Ineffective (because client has a weight bearing as tolerated, from the
fracture sustained on her right femur)
So... For level 2, i would have to clearly label the focal, contextual ..
So is it like: Client has weight bearing as tolerated on her right side, related to the unspecific fracture of the neck of her right femur?
but isn't that the same as a nursing diagnosis?

i don't believe you'll find "weight-bearing as tolerated" in any list of nursing diagnosis, because that is part of the medical plan of care (what some people call "medical orders"), in order to decrease mechanical stress on the recently-discovered fracture.
what's the nursing diagnosis? there are several. think about her balance, her strength, her pain, her nutrition, whether she knows why all this is happening-- do any of these suggest "risk for ..." or other deficits addressed by nursing to you?
