Participant Invitation Letter

Dear Prospective Participant,

Thank you for your consideration in completing this survey. The following survey is being conducted for an Applied Research Project (ARP) in partial fulfillment of the requirements for the Doctor of Health Sciences degree at A.T. Still University. The purpose of this study is to identify perceived barriers that prevent staff nurses from contributing to evidence-based practice (EBP). This study is of high importance because without identifying the specific barriers, there is no way to address the problem.

You have been chosen for this survey because you are a provider of direct patient care, and have knowledge and insight into the best outcomes for the patient. This survey is completely anonymous and your honesty is extremely valuable to this research study. There are no personal identifiers collected within the survey. The completion of this survey should not take much of your time, since it can be completed in approximately 10-15 minutes.

If you are a nurse who performs bedside care to patients you are eligible to participate in this survey. The survey is intended for staff nurses only; educators, managers, and administration are excluded from this survey.
