I had spent time answering these questions and when I went to reply you deleted it. Here is what I had answered:
3. D (pneumonia=fluid in lungs=crackles).
6. D (I believe these are normal and should improve. As long as they are voiding it is okay).
10. A (to prevent that 90 degree rule for post-hip surgeries).
18. C (I remember reading this somewhere, although I have never worked in dialysis so not 100% sure).
19. C (I believe the Hindu culture prohibits beef as they believe Cows to be sacred).
30. D? I assume this because of the most common signs of stroke, but knowing which kind would help.
32. C (Asking each time ensures an adequate assessment and allows you to establish trends in pain levels).
33. A (60 mls won't cause any harm unless it's been a long time with no voiding. I would follow the doctor's order, or unit policy as it will usually state when to contact a doctor with a post-residual volume amount). I would document and pass along the information to the next shift or repeat the scan before you leave!).
38. C (This is the only answer that makes sense. Hourly assessments of a patient in restraints needs to be done by a nurse, patient teaching about fall risk plans needs to be done by a nurse, and speaking to family also needs to be done by a nurse).
