Would any of you be willing to share your ROI forms? We are thinking ours is too vague so we'd like to rework it.

Not sure if this covers it for you but this is the one I had to sign for my DD's school and the Lead Nurseand I were commenting how it is hidden among everything and don;t think parents really understand what they are signing. The one is the district where I work is better, but the website is down so I can't access it for you. Here is the one from my DD's district- see if you can find the ROI part, LOL.
