hi im new, could someone help recommend some good nursing care plan books to me please?
ones with nursing diagnosis with interventions and rationales for each?

I really enjoyed Ackley & Ladwig, Nursing Diagnosis & Care Plan (I believe that's what it was called)
This book has tons of interventions & rationales for each, couldn't have done my care plans without this book!

Sorry for not exactly answering your question. But here's another way of looking at care plan resources. I found care plan books were pretty much the same from one to another. I used the highly portable and easily reviewed Nurses' Pocket Guide (Doenges, et. al.), then used the Merck Manual on-line (free and very informative) to substantiate interventions. This ensured they mapped well to the medical and pharmacotherapeutic interventions. It will require extra time to make a nursing care plan, but you'll learn more. Also, as has been advised by others, try to make sure you do your care plan in some electronic form (e.g. Word). You'll be surprised at how many times you'll use and reuse parts or all of a previous care plan.

I agree that Ackley is great. You can look up nursing diagnosis' based on medical diagnosis'. Also, all of the interventions are listed with rationales and they give you outcomes to work from. I used this book all through my first year!

I found a book called "Lewis's Medical-Surgical Nursing" to be absolutely amazing.
It taught me a lot about my care plans. Give it a look.

in response to lewis med-surg and ackley suggestions, our curriculum requires them both. if you go to evolve, you can look up ackley and lewis titles, add them at no charge, and use the resource portion. ackley's on evolve offers a care plan constructor - point and click with outcomes and rationales. save $$$$ other good additions for care plans in a more user friendly format is [color=#003399]nurse's pocket guide: diagnoses, prioritized interventions and rationales (nurses pocket guide) by [color=#003399]marilynn e. doenges and [color=#003399]all-in-one care planning resource: medical-surgical, pediatric, maternity, and psychiatric nursing care plans (all-in-one care planning resource: med-surg, peds, maternity, & psychiatric nursing) by [color=#003399]pamela l. swearingen. can a nursing student ever have too many ebp resources?? one more piece of advice is to make extra copies of your care plans and keep one in your clinical folder to reference. i am req to turn in my care plan after clinical and have in the past not received them back -make a backup copy for yourself! good luck
