When I was the steward (CEO) of Dynamic Net, Inc. we found a reasonably priced developer using the above four groups.
If you are in the U.S. I strongly recommend using only U.S. based people who do not outsource any of their work overseas.
I strongly recommend you check designer references; AND, you make sure there is a procedure in place that you receive proof of purchase and copyright use paperwork for any images used on the site (I've personally witnessed horrors where one of our clients used a designer that could not provide proof of purchase & copyright use where our client was sued -- and lost -- for double digit thousands of dollars; it could have been more if the law firm on the other side thought they could get more).
While most designers use an hourly rate, they often invoice by the project; and you want me make sure of the project phases as well as milestones.
One benchmark is to take the overall project cost divided by the number of pages for the site. If the per page price is less $50.00, then something is fishy (i.e. like seeing a gas station offering gas at $1.00 per gallon where other stations are charging $5.00). If the price is more than $500.00 per page, then that's probably too high unless you have a lot of specialized application (which is not HTML web pages) being done to integrate with databases.
