"regulated health occupation"

Hi everyone!

Just a quick question in hopes that someone can help me so I can mail my State Board of Nursing Application in tomorrow morning! There is a section on my state board of nursing (Indiana) that says "List all states, including Indiana, foreign territories, or countries, in which you hold or have held a license, certificate, registration or permit to practice nursing and/or any other regulated health occupation."

My question is: I have been a certified nursing assistant since sometime in '98 is this considered part of that?

My state's Aide registry got hacked this past year so they took down our information and we are unable to view via web our date of issue for CNA licensure so I'm in a bit of a quandry.

I've got my registration number, but no date if issue... I know the month and the year, but not the exact date. Do you think this is something that I should wait and call about Wednesday or do I even need to be worrying about it?

Thanks to anyone that can help, I finally get over my psychological block about doing all of this and now I have a ton of stupid questions!! :banghead:
