I am kind of trying to sort out the differences between "community-based alternative" and "nursing home".
Community-based is the one de-institutionalized.
Nursing home is the more like living in a facility.
However, what the community-based alternative would look like? and what Rn can do over there?

Community based = pt lives at home. May go to adult day care, senior center, have in-home assistance. Family/caregiver is responsible for meds, etc.

And as I understand from the internet, individuals who are entitle Medicaid now can chose where and how they receive the medical care. For example, in the nursing home or in their private home.
Here is another quesiton though: How is "community-based alternative" different from home health? Or these two are just under different reimbursement system for LTC?

Neither of these is the equivalent of LTC. People who live at home go to their own doc; in some cities there are docs who make housecalls for those who are truly homebound - confined to bed or chair, unable to be easily transported. There are two such services where I live.
Some patients may receive home health services; an aide who bathes them 2-3x a week, for example. Or if they need wound care, or med monitoring, there may be a nurse.
But the vast majority are taken care of by their families - spouses, siblings, children, etc. Many of whom manage very well, some become overwhelmed and need assistance. Every case is different.
People who stay at home have nothing to do with LTC facilities. And the facilities may decide if they can accomodate each individual according to their personal and medical needs, as well as their finances.
