Is impaired verbal communication a psychosocial or physical problem? How about self care deficit/bathing and hygeine??

I am still a student too but I think both dx's can be attributed to either a physical or psychological problem. You'd have to see how it applies to the patient. (Is it a physical reason Self-care deficit r/t pain or pscyh Self-care deficit r/t impaired thought processess)

Thank you so much for your reply.....It's self care deficit related to altered mental status so I am going to use it as a psychosocial.

Thanks for all the suggestions guys....I ended up changing it to chronic confusion r/t impaired cognition because this is the last care plan I have to write this semester if I get a Satisfactory on it and I did not want to take the chance of it being physical.

This might be a teeny bit of topic, but is relevant in a way. We have 4 care plands each semester to do, but if you do good on the first three you may be exempt from the 4th one. Well, semester one was a learning experience for me and had to do all 4. Semester 2 still learning and had to do all 4. Now semester 3, still learning, but had learned enough about care plans and what my instructor wanted to do good on first three. she gave me my third one back and told me i was exempt from doing the 4th one. There is a care plan god up there. Since i wont have to collect all that info for a care plan, what does she have in mind. We will be given 3 patients to do total care on, from baths and linen changes to meds. This is what i think clinical on the floor should be like, not digging thru Dr's orders, trying to read his orders for the past 4 days, just taking care of the patient's total care for that day. Will be looking forward to it.
