Quick question on cerclage management....

Hi everyone, and HAPPY HOLIDAYS!!!!

I have been MIA for a couple of mos d/t re-location, etc, etc.

Started a new job, in a military hospital working hi-risk L&D, however I float occasionally to the PSCU/Antepartum unit.

My question....What is the usual management of post-cerclage patients? How long do you see them kept in house? At what point, if any, do they go home? What is their "usual" management? Bedrest til end of pregnancy...say cerclage placed at 16wks......flat in bed til 34 wks?

Now I know that circumstances do vary, and a lot is dependent on what the womans' case is, but I'm looking for general guidelines....

I'm flying "home" this morning to spend Christmas w/family!!! Yippee!

The majority of cerclage pts. go home the same day the cerclage is placed, and stay off their feet for at least a couple of days--then they are on limited activity and typically monitored every two weeks to check cervical length up until 24 weeks. Although some pts. are able to continue working through their pg, , the majority end up on bedrest by around 28 weeks.

In my personal case, I was placed on strict bed rest at 23 1/2 weeks and by 26 weeks the stitch was the only thing holding the baby in. I made it to 33 and the stitch would have held longer but for chorio secondary to GBS.

Usually it is placed as close to 12 weeks as possible--in my opinion 16 is too late and some women are already starting to change by then.

I am a firm believer in the cercage and so thankful for all the healthy babies it has helped.

I have been working high risk PSCU in addition to L&D lately....hoping for thoughts from other high risk OB nurses....

We place tons of cerclages,....curious as to in-house management, etc.
