What would happen if a dialysis line locked with sodium citrate was accidentally flushed before drawing back? Is 1.5 mL of sodium citrate enough to cause patient harm?

No generally it is not enough to cause harm really. But an incident report needs to be completed.
We all err; you won't have this mistake on your hands again.
