Greetings! I am a PT not long in homecare, still learning the ropes. My agency has no policy in place currently regarding PT only cases and the discharge oasis. RN does Soc. We had a patient who was PT only, the PT spoke to the supervisor in the office on the phone so the supervisor could answer the oasis. The supervisor fills out and signs the oasis but writes 'written with consultation from PT Smith'. Is this ok?
What are your agency policies regarding PT only and oasis discharges?

In a word - NO. The admitting clincian is the one to perform the SOC OASIS assessment. If it is PT only without orders for nursing, then PT can do the OASIS. If company policy is to have nursing do all SOC, then the RN need to be the one in the home doing the assessment - end of story. The assessment is to be done by one clincian only. They can confer with you and your findings, but the assessment must be theirs. Otherwise you are NOT following Medicare COPs. The last qualified clinician in the home should be the one to do the OASIS d/c assessment.
