Hello Everyone,
I am a recent new grad RN, and I have just started working as a psychiatric nurse with a Community Treatment Team, the majority of my patients are schizophrenic. I am hoping that some of you may be able to help me coming up with a good assessment form (I will be going to be visiting them in their homes and out in the community). The current RN on the team is transitioning into another position in another facility and I am going to be the only RN on the team (I will be working with therapists, vocational specialists and a psychiatrist). I want to come up with a stream line assessment form focusing on the nursing aspects but I do not know where to start (my co-workers focus is some what different than mine). If anyone has any suggestions for me that would be great!

I couldn't open the link, but here's what I think, I would include:
MMSE
Non-verbal
Suicidal/homicidal/depressed/anxious? (if so, scale)
Changes in medications?
Medication compliance?
Dressed appropriately for season?
Affect (does it match what they're telling you?)
VS
Use quotes from the pt. whenever possible!
