Protonix IV push rate?

OK so, I feel like the "What's heavier a kilogram of steel or a kilogram of feathers" guy right now.

At clinical, we were ordered to give 40mg of Protonix IV in 10mL. When I looked up the push rate in Lexicomp, it says to push 4mg/mL over 2 minutes. I thought, "Surely we're not pushing this over 20 minutes??" My instructor told us no, the entire 10mL will be pushed over 2 minutes.

I'm not understanding the logic of this. I know 40mg/10mL reduces down to 4mg/mL, but since we are giving 10x the amount listed for the push rate, why wouldn't the push rate itself be multiplied by 10? WellRx seems to agree with my instructor, and this is what it says:

"-Slow IV push administration has been used for adult patients.
-Dilute each 40 mg vial with 10 mL 0.9% Sodium Chloride Injection. Each vial will have a final concentration of 4 mg/mL.
-Infuse slowly over at least 2 minutes. Do not give by fast IV push."

I know the explanation is probably super simple and I'm going to kick myself over it, but can someone please explain this to me like I'm 5?

We had an IV pump that allowed us to connect the 10ml syringe to deliver the medication as a secondary infusion. The facility policy at that time was to deliver Protonix over 4 minutes (anything over 2 minutes had to be placed on the pump). Literature, research, and subsequent policies change/evolve as time goes on...hence your 2-minute administration of the same drug. Questions like the one you're asking were always asked of the inpatient pharmacist if I was unsure or just wanted to inquire. Nowadays, protonix at some facilities comes in a 50 ml piggyback bag. If I am going to push protonix through an existing infusion, I just push it normal. The running fluids will dilute. If I am just pushing it, I do the 2 min.

We have had issues with supply, so usually it comes in a pre-mixed bag. When we are out, we have to reconstitute.
