please help MDS Coordinators
you have to look at the overall picture of the resident and whether or not the situation is limited like a resident who is normally ambualatory and gets the flu during their assessment window and spends most of the week in bed or a wheelchair. while they likely met the two or more changes for a sig change they are expected to return to their prior level of function once they are over their illness. so then i would likely not do a sig change. another area where you only need 1 criteria is a resident who has been total care maybe even comfort measures they have no real change in mood, behavior, pain, cognition or ADLs but family wants hospice then you are required to do a sig change even thought the resident has not acutally experienced a new decline. Hope this helps

Sound like a good job offer?
if it something you are interested in doing and the drive isnt too much then I would say go for it now days it is becoming more difficult for nurses with no MDS experience to get in to the field. So if you have the opportunity, the money is right and you work out all the details like hours and responsiblities and can get the training you feel you need I would go for it.

assessments
did you do a quartely with your readmission/return MDS as the resident would have been due for their first quarterly which on med a res I usually combine with my 90 day assess or in this case i would have done it with my readmission/return mds. if you havent done the quarterly get it done now so you are not out of compliance and then the resident just reverts into your obra schedule of quarterlies and annual. As for resident #2 if you discharged them return anticipated you only have to to a 5 day mds unless they met the criteria for a sig change then i would do it as a sig change/5 day if you discharged them return not anticipated then you treat them as a new resident Hope this helps

Why Does Everyone Hate Florida So Much??
Well I for one dont get it I decided to move to florida in march so I visited for the first time in june and I loved it. My son and I moved to florida in july and so far it is awesome I was able to find a job before I moved that paid better than what I made in Minnesota. I have a great little two bedroom apartment and so far the people I met have been really nice. I am now looking into going back to school to get my BSN. The only draw back so far has been schools for my son they have you fill out forms and list where you want to go but then they put you wherever they want anyway. Which leads to some unhappy frustration among a 16 year old who had his heart set on another school but he will be fine. I love my job and i work with great people. I have no fear of living in a city this size (the same wouldnt have been true in minnesota) yeah its warm but I dont miss the cold and I definitely wont miss the snow.

Best/ Worst Places to Work Near Punta Gorda.
I too am in the process of getting my florida license and have probably spent close to 400 in fees and postage. I am seeing alot of jobs but most wont even respond until you have the florida license I do have one company sending me an application as she liked my credentials and I did tell her that I had already sent in all my paperwork to the board. My only concern right now is reading all these posts get me wondering about pay, but i already have the apartment lined up and i will have the moving company hired next week. I have a friend moving there in june who is going to continue to help with my search and to at least get my foot in the door i am willing to take any job and if i needed to any shift at least until i am done moving. good luck to both of you in your search. What areas are you looking to live and what type of nursing do you want to get into if i may ask

RAPs
so what exactly is your question? When I write raps I start my rap by writing what triggered the rap and then i write out my reasoning as to why I do or do not agree with the trigger and whether or not I will write a care plan (does that make sense) surveyors have not had any issue with my raps and my don trusts that i know what i am doing so she doesnt double check either. Everyone needs to develop a system that works for them.

travel nurse resume
Well since as a travel nurse you technically work for the agency. I would put the agency down and then list the different departments you worked as well as pertinent clinical skills. In an interview you would be able to further explain all the different hospitals

Jobless and Frustrated
where in minnesota do you live? have you considered anything besides hospital work (like home health, Assisted living, agency nursing(esp with you ltc experience)
-
Life Insurance/Long Term Care Insurance Assessments
where do you live? I do know of one company in minnesota. I would like to get into casemanagement but am having difficulty finding a company that doesnt require a bsn
-
ltc facilities
if you go to medicare.gov you can do a nursing home compare and that will give you all the LTC in whatever city or area you are looking for

Need Help! Testing out of CNA Course in MN?
I have not heard of being able to take the test without the course but if you could I am not sure I would unless of course you have worked in the health care field as a pca (then you would have most of the skills) I agree that I probably wouldnt pay for a course I didnt need. Are you going for your RN or LPN because I had heard that Pine tech in pine city was going to do away with the CNA requirement. best of luck

Accepting two job offers then turning one down later?
you have to do what is best for you. But being as the second job wouldnt even start until september I would accept both jobs (what happens if you move start the new job in july and hate it after a month you would at least have the other job to go back to ) In this economy the employer would be able to fill the other job even with only a months notice (heck most would understand that the job doesnt start for several months and not everyone could afford to be unemployed from now until september heck most couldnt afford to be unemployed until july. I say go for it you never know your ideal job may not turn out to be that ideal. you are in a good position as a new grad to have two positions offered you that start two months apart but that is MHO:wink2:

does LTC work count as experience?
I dont see why it wouldnt. I have heard that sometimes hospitals like it when nurses come from long term care because they are used to multi tasking. where as i have seen the opposite when a hospital nurse comes to ltc.

MDS 3.0

New RN BSN over 50 and Can't Get a Job
sorry to hear that you are struggling. I would consider a long term care or home health agency at least to get your feet wet you can always continue looking and applying to hospitals. I am not sure where you live but i wish you luck

Recovering Nurse Jobs
Was your license affected by your addiction? If it was not affected how do potential employers know about it? Have you looked into non hospital/clinic nursing ie case management, insurance, telephonic interviewer etc? Good luck to you addiction to anything is a tough thing in life to overcome and you have likely have a long journey in front of you

Living in WI
it is my understanding if you get your CNA in wi that minnesota and wisconsin have a reciprocity agreement and that you just need to apply to be on the minnesota nar registry. Minnesota does not charge to put you on the cna registry but wisconsin may charge to process the paperwork. I am in the process of helping my aunt who lives in wisconsin and is a new cna but would like to work at the facility i am at in minnesota as a cna. hope this helps brenda

Responsible for being on call ?
I am on call every 4th week for 24/7 for the week. I would have to come in if absolutely needed and I have also helped cover 1/2 shifts from nocs into days if we are short noc nurses. I dont get paid extra cause i am salaried but i will try and leave early ie if i start at 3 am then i try to leave at noon. if i cant that day then i leave early another day. where it gets really frustrating is when state is in the building as i am required to stay as long as they are in the building one time i started at 4 am and because of the number of assessments i needed to complete i couldnt leave early. and when i was trying to leave at 4 the state walked in they stayed until almost 11 pm that night needless to say at the time i lived over an hour from work and i knew they would be back by 7 and i was on call so i spent the night trying to sleep in the therapy office. (staff trying to be cute and funny because it was april first woke me at 3 am saying state was back) other than this i love my job 99% of the time

Staying for Meetings after Your NIGHT SHIFT!
my experience has beent that it doesnt matter we schedule meetings at 730 so nights can stay or 10 so nights can come in early and time and time again they dont show. days complains when the meeting is during their shift because they dont want to stay for the on on the pm shift and pms complains because they have to come in early to go to the one before their shift. its not like managers hold meetings every day my don tries very hard to be accomadating to all shifts yet time and again they whine, complain and dont show and then get mad when they get written up for not attending a mandatory meeting. I scheduled 3 different CPR classes for three different weeks at three different times and only had 11 staff show up only 3 of whom were nurses the other 10 who signed up didnt show and now they are whining because their CPR is expired and want me to hold a special class ( i dont get overtime and i dont get paid to teach this class it costs me 4 hours out of my day and being the mds coordinator that is huge. So I think people just need to stop whining and attend their meetings sorry if i sound like a huge b**** but this whole thing irritates me to no end

Surveyor thinks I miscoded section E
It is probably not worthy of a deficiency however surveyors will find deficient practice where they want to. The social worker made a note in the chart about her concerns. You witnessed these complaints (did you make a note). I would also get out the section from the rai manual and show the surveyors also did you interview the nars (as typically they dont chart in the chart) on your mar did the nurse giving the med for insomnia write why they gave it? I would not particularly worry too much if you have any documentation to back up your charting I would however reiterate to the nursing staff how important it is to make nursing notes for all prns and for any resident health concerns
