Shingles
You forget there are a lot of older teachers and nurses for that matter still in the work force :) I have had a family have the vaccine only to both contract chicken pox. Doctor said they were fine and mother sent them to school. I saw th and sent them right back- he said I was right. They both had 2 vaccines.. so I always err on the side of caution

Shingles
I would be concerned. I have students that are immunocompromised, religious exemptions and pregnant teachers. We have a family member in his 30s that did contact chicken pox from his mother who had shingles. I was pregnant working in a ER and my titers were very low. I never took care of someone with shingles. Finally hospital policy made me get two varicella vaccines in 2010. We had a nurse that I worked with that was immunocompromised and would break out frequently and regardless if it was covered or not- could not come back and provide patient care until it was healing.

Lice policy?
we are still no nits/lice policy and have to go home.. Its a tough one. I check, call parent, encourage pick up. ( If not braid hair and they stay in school until dismissal) I recheck in the morning upon returning to school. If it looks like it was treated and nothing is live and few dead nits- I let the principal know and its his call to make. we do sent a general lice letter out the class affected.. I do not make classroom calls just because.. If I do get send one because at teacher says to check them- I will and call the parent to let them know.. Let me tell you nothing like a upset parent stating you are picking on their child.

That's the third kid this morning
Write up a small paragraph about what is required for medication in school- including over the counter. Have you school put it in their newsletter online or on paper.. Hopefully parents will read it and understand that its not safe to give their darling children medication to bring to school and that they cannot take it without a doctor's order + it stays locked up in the clinic

No field trips either here.. Nurse is required to be on campus.. I like the fanny pack idea. I'm sure that would come back to me..

Students using cell phones to call home sick
It happens also to me in middle school. If they don't come to see me first and call/text parent for pick up- they have left the school nurse out of the equation. They do not wait in the clinic for pick up. They can go and wait in the grade level office. I have parents call and ask if they have come in because they are texting them to go home sick. I tell them the same thing- Now if they want to include me in their illness- they are more than welcome to come to see the school nurse like they should in the first place. I tell my own daughter that in high school- stop texting me and go see the school nurse. After her assessment- call me..

Camp nursing in summer
I have done it also over the years. I like the college camps the best. Nice room. air conditioning and their numbers seem lower. Plus shorter sessions. I'm not into my whole summer being away at sleep away camp. I felt like I was working in a ER. I swear some days we saw 300 kids plus meds.. Crazy. But if you can find one that is well run, great nurses to work with and making money- it will be a good experience

Does your district allow narcotics in the clinic?
I think that will change with the newest FDA guidelines coming out regarding codeine and tramadol. ( I have never seen tramadol prescribed for a pediatric patient )

I have to ask- are you really seeing MD prescribing Tylenol with codeine anymore ? Codeine is contraindicated for pain in children. I didn't like it when I did ambulatory surgery and recovered kids and that was years ago.. now they are adding Tramadol to this list.... i know my daughter is a ultra metabolizer via genetic testing years after her T/A. She had a terrible time with Tylenol and codeine after that surgery- switched to two days of roxicet and she was fine.

Applying for a new School Nurse job...
We have some two nurse schools that use LPN with a RN. They do the office visits and meds. The nurses do the case management.

You are going to be busy... just be prepared... long days. We have had two nurses go there and not return.

Wow is your administrator school based? I would think they would want them in class learning.

I look at it like my post op days- narcotics are for home use.. Too many liability issues at school, side effects and than there is the daily count that should be done by two RNs. That's what we had to do every day in recovery. anyhow- I would never advise an adult post op to drive under the influence so why would I think its okay for a pediatric post op at school? Its come up like once in my school nursing career so far

wow that is a lot of office visits. salong.. I have 600 and barely see 20 a day. I'm not responsible for clothes changes, etc.. front office handles that. I don't like to cross contaminate... so sick kids only.

end of year sending meds home
our policy is a parent/responsible adult picks up the medication over the counter or prescription. I send home a letter mid may. Most of the time- I do get parents to pick up. Other times they think leaving it during the summer is okay. My office staff that works in the summer have access to sign it out to parent. I do have on occasion left over meds come fall and I'm told to try and contact the parent before wasting it even though its our policy to waste if parent hasn't picked it up..

Interview questions to ask Camp Director
do your nurses generally come back the following year? (that way you know if its high turn over). How many campers per session, etc?Where do you sleep? air conditioning? wifi? background of other nurses that you will be working with? Medication policies- Camp Meds? or can they just bring them ? what type of chronic health conditions do the campers have? protocols for emergencies on camp? and are you on for 24/7 or is there shift work? Do they reimburse you for your travel/ RN license? these are just some of the examples I ask.

Would you call an ambulance?
No as long as the student was hemodynamically stable- I would call parent and recommend medical f/u. As far as principal goes- its their school- so if they want to override the nurse- that is their choice to make. I just make a note in my office visit on the outcome. Once EMS arrives care is transferred to them and they can make the call. Parent can sign refusal to transport and take the student home at that point.
