Procedure for accessing a Portacath

There are a couple of things that should have been done differently:

1. The area that was cleansed prior to accessing was not large enough. Therefore, once the drsg was placed, there was skin under that dressing that hadn't been cleansed!

2. Ideally, the kit should have had a sterile fenestrated drape put on prior to sterile access.

3. The pigtail of the port needle should have been clamped BEFORE removal of syringe and attaching of injection cap.

But other than those issues, Teddy and Chester looked like they were having fun!!

Great job!

The only thing is when making the dressing truly occlusive at the end,

a piece of tape should be notched and placed under the needle extension at the edge of the dressing and another piece on top of the tubing directly over the piece just placed under the tubing. Otherwise there is still the possibility of bacteria entering under the dressing.

One thing....I would have washed my hands again after placing masks on the patient and visitor, prior to donning sterile gloves.
