Please go home...you are stable enough to be discharged...don't make me call security
For my regulars who are difficult to discharge, I make them feel very comfortable. Warm blankets, quiet tranquil environment, dim the lights. Basically roll out the red carpet. When it's time for discharge, reverse everything in about 2.5 seconds. Lights come on, blankets come off. Loud booming voice explaining discharge instructions. Position the bed in high fowlers position, IV out, vitals done. Basically make it so that going home is a really good idea right now. I want to emphasize that I do this only when necessary.

Whats with Mondays?
Sometimes people are not sick enough to warrant going to the doctor's office, but work requires that they have a doctor's note if they miss work. Some schools are now requiring a doctor's note for missing school. Seems ridiculous to me.

I'm thinking anxiety related. He blew off all of his CO2. Some COPDers live with a P02 of 60.

What have you found in your pockets at the end of your shift?
One time I was home stripping my scrubs off and I found several unlabeled blood vials with blood in them. Good thing they didn't make it to the laundry :-P

Nursing school prepares you to start learning as an RN.

Morphine dose in vial?
Ask your pharmacy for smaller vials. That would be a huge waste of time trying to find a witness for 80-90% of my morphine pulls.

Need advice, want to work in a cath lab!
Now I'm 6 months into my ER RN job and I still really want to get into a cath lab. I plan on staying here for atleast a year. What should I do next? Do I need to work in an ICU before transitioning to the Cath lab?

So the second set of BCL is important enough to be accessing EJ's for them then? Or art sticks? They are that important?

Ok. I have just discovered the search function and found some good posts re: the WHY of cultures. But the question remains, when should I be standing up to the white coats and saying "I can't get the second set of cultures, she's been poked multiple multiple times, can I just start the ABx anyways"? Thank you

How important are these for patients? I notice doctor's will order these on most every patient that gets admitted. How important is it to get that second set? In normal circumstances I don't mind pulling them but let's say I have a patient who is very ill and has ABx orders, but they are a difficult IV stick. They have a 24 gauge in the pinky finger and NO other veins. Should I be killing myself and poking this patient a million times to try to get the second set? Please and Thank you.

Greetings! I am a new grad RN who's worked as a Paramedic for the last 4 years in the Emergency Department. How would I go about getting into the cardiac cath lab? I just signed a position in an Emergency Department as an RN to get some experience, but would like some guidance from you folks! Thanks, Garrett
