Problem focused office visits - ideas for forms?

Hi, I am in primary care clinicals for my FNP. My preceptor doesn't like the form she currently uses in her practice because it is designed for a complete H & P visit. We looked at the various forms that can be purchased, but they are too general. She asked me if I could come up with a form that could be used for an episodic or problem focused visit. I have been looking at forms from other practices and have a rough draft, but I would like ideas (from those of you who are currently practicing) as to what sort of form works for you.

This books has forms for everything, including problem focused visits - it comes with a disk that has all the forms in word and acrobat formats that you can modify.

The Family Practice Management (from AAFP) has a "toolbox" section that has all sorts of forms to for practice and to run a practice. Several of their clinical forms are encounter forms that are specific to the issue being dealt with. Hopefully you'll find what you are looking for, or at least generate ideas for your form. If you design one, I'd love to see it. Am always looking for good forms.
