potential and actual diagnosis for pt. with CVA bleed

i am really really confused right now... first, it's my first time to encounter patient with CVA.. she has history of hypertension, and i think that's one of the risk factors.. worst, i only had 2 hours monitoring her because, she died.. the worst part is that, my C.I insist that i should make an NCP out that patient.. when i first received the patient, she's unconscious, with IVF, NGT, O2 and she had a fever of 39.9..

i was just confused on what actual and potential nursing diagnosis i will make...

please help me, because if i have a nursing diagnosis, it will be easy for me to make the rest..

A lot of really supportive people posted in your previous thread about this. Look at the links they gave you. Think about the information that you were given. You can do this. But the idea is that YOU do this. We all did, and it was fine. Hash out your care plan and discuss it with your CI to help tweak it. Some instructors are very good at this.

Also, to add to a previous poster on your other thread about the exact same care plan - they are busy work-ish, but it does train you to think "like a nurse".

I agree with the other posters, you need to do this work, in the same amount of time it took you to right this post, you could have looked up some nursing diagnoses for this patient. I'' give you two potential ones

Alteration in skin integrity R/T Immobility

Potential for Impaired Respiratory ability R/T Aspiration
