I am needing some advice. I have a ppt pt. that will be 24 hrs post op in the morning. I have a #1 diagnoses of risk for fluid volume deficit r/t blood loss during surgical procedure and possibility of post op complications. Out come #1 patient will maintain adequate fluid volume #2 patient will remain free of symptoms of Fluid volume deficit #3 patient will remain free of post operative complications. What I am trying to do is tie in the possibility of hemmorhaging or post operative bleeding into my dx of fluid volume deficit as blood loss assessment is a part of our BUBBLE assessment. Does this sound alright or am I stretching outcome #3 too far?

A post op patient is going to have an incision that needs attention and pain. How will you address those problems? They will be actual problems. A Risk for Deficient Fluid Volume is an anticipated problem--it doesn't even exist. She may or may not have voiding problems as a result of anesthesia complications. If she is breastfeeding there is a diagnosis for that.
