please help me with my nursing issue and goal, if i write it well... pt had a frequent falls, and poor vision.

Nursing Issue: Pt. has a risk of fall related to disturbed sensory perception visual .

Goal/s: To prevent further risk of fall and take measures to restore disturbed sensory perception vision within 2 weeks as evidence by disturbance in balance/ frequent fall.

You lack 411 to help you - but this what I can attribute:

Distrubed sensory percetion R/T sensory reception AEB documented loss of vision. Allowing patient to voice fears aid accepted of vision loss.

`Risk for falls R/T poor enviromental safety aeb 'of past hx' Patient and family are able to point out things/issues in the enviroment that put them at risk.

I think your goals may be focused on something you can measure.. correct?

Is patient having frequent falls because they can't see? Are there other reasons? Abnormal gait? blood pressure issues? Need more info
