Can anyone direct me to where I can find care plans for a newly diagnosed, mentally ill (schizophrenic) student? At the very least, I'd like to get an individual health care plan in before the end of the school year, with an eye on an IEP for next school year. At this time, the student is functioning on meds, but I have to look towards the times school might be missed due to hospitalizations, counseling & doctor appointments, etc. I'm not so sure all the teachers understand the chronic nature of the illness, and its effect on learning.

You might want to get in touch with your school's psychologist. They should be able to steer you in the right direction. Good luck to you, I'm sure this will be a real challenge.

There is a care plan for Bi Polar in the following book that might provide a template:

You should work with the parent and the provider. For MH disorders should be very individualized.
