Please help ASAP. :uhoh3: I have a pt who has tube feeding and I have a nursing dx of imbalanced nutrition

can't be of any help to you because you haven't told us what the patient's symptoms or any of your interventions for the imbalanced nutrition less than body requirement are. goals are the predicted results you expect to see when your interventions are performed. without knowing what your interventions are it would be silly to suggest any kind of goals.

can you use ineffective protection if a pt has an infection somewhere in the body? what would the goal be since you can't say "remain free of signs of infection"?

ineffective protection
if the patient already has an infection. if the patient already has an infection they are at risk for sepsis and the diagnosis to use for that is
risk for infection
meaning sepsis. your interventions are

  • strategies to prevent the problem from happening in the first place

  • monitoring for the specific signs and symptoms of this problem

  • reporting any symptoms that do occur to the doctor or other concerned professional

and your goal is to prevent the patient from developing signs or symptoms of sepsis.
