As a seasoned RN just reentering hospital nursing, my organizational skills need some tweeking. Does anyone have a worksheet example for taking and giving report? I am on a pediatric unit with some adult overflow. I work nights and usually just have 4 pts. thanks for all the help I have gotten from this forum. Susan

When I worked ICU stepdown I made my own brains in excel. I had been copying the worksheets other nurses had, tweaked them a little bit and came up with one that worked well for me. If you PM me your email address, I can send you the file. It's set up for 4 patients on on page, space at the top to stamp the blue plate, times for meds (so you can just circle when each pt is due for scheduled and prns) and accuchecks, space to write labs and sections for each system (neuro, cardiac, resp etc)
